Project Transitions is the only Central Texas organization providing low and no cost hospice and recuperative care, transitional housing and wrap-around services for people living with HIV/AIDS. What started as a small hospice facility in 1988, has grown into a community and hope for those who have struggled with illness, stigma and lack of support.
Doug’s House opened in 1988 to offer traditional hospice services to those with HIV/AIDS. It’s mission has evolved to include short-term intensive respite care as the treatment for the illness has improved. In 2013, 13 of our 14 patients walked out to resume their independent lives. Doug’s House provides 24-hour care to up to 5 individuals from social workers, caregivers and a registered nurse. No matter where the next step in a resident’s life takes them, our hospice provides crucial support services to help them through their life journey. These include:
End of life planning/support
Medication Management
Transportation when possible
Meals
Family Bereavement Counseling
Medical Care Plan Development
Resident Memorials
Connect clients and family members to case management
Medical service coordination
Roosevelt Gardens, Highland Terrace and Community Housing provide affordable, transitional housing for HIV+ individuals and their families with wrap-around services by professional social workers. Community Housing provides subsidized rent assistance and support services for those living offsite. Roosevelt Gardens and Highland Terrace offer one and two bedroom apartments in North Central Austin. Services include:
Low-Income Subsidized Housing
Help locating permanent affordable housing
Life skills, money management & independent living skills training
Informal mental health & harm reduction support
Nutritious community breakfast Monday through Friday
Vocational Guidance
Top Drawer Thrift provides approximately 1/3 of our operating budget and has been named “Best Thrift Store” by the Austin Chronicle. All proceeds from the sale of donated merchandise benefit Project Transitions’ programs and services. In addition, Top Drawer serves the HIV/AIDs community through the provision of store vouchers distributed through area HIV service agencies.
